Monday - Friday flexible between 7a - 7p (OT may be required including weekends)

ROLE IMPACT
The Patient Care Coordinator is a critical member of the support team, ensuring seamless communication between patients, providers, and internal staff. Success in this role is measured by accurate scheduling of medication shipments, timely documentation, and compliance with healthcare regulations, directly impacting patient care continuity.

KEY RESPONSIBILITIES
• Answer incoming calls and provide timely, accurate information to patients and healthcare providers
• Contact patients to gather necessary details for coordinating initial and refill prescription deliveries
• Process and schedule medication shipments with attention to accuracy and timeliness
• Maintain patient records in compliance with HIPAA and organizational policies
• Gather and report clinical data to the care team, including lab results, assessments, and hospitalizations

MINIMUM QUALIFICATIONS
• High school diploma or equivalent
• 1–3 years of experience in a customer service center
• Strong telephone communication and multitasking skills
• Understanding of HIPAA and compliance protocols

CORE TOOLS & SYSTEMS
• Multi-line phone systems
• EMR/EHR software
• Prescription and claims processing platforms
• HIPAA-compliant documentation systems

PREFERRED SKILLS
• Prior healthcare or pharmacy operations experience
• Knowledge of third-party claims adjudication
• Familiarity with clinical data collection processes

EQUIPMENT & WORK ENVIRONMENT
• Office-based setting with phone and computer use
• Must be able to sit, stand, walk, and reach outward frequently
• Ability to lift and carry up to 15 lbs; occasional pushing/pulling up to 15 lbs
